超过约1千万的儿童年龄在五岁以下，每年都有近半数儿童在出生后数周内，死于可预防的疾病和营养不良症。
在某些发展中国家，大约每五位儿童中就会有一位在五岁之前死亡，死亡率之高令人震惊。那些幸存下来的儿童也不能得到全面的成长和发展。大部分死亡是由下列五种原因，或某几种共同导致：急性呼吸道传染病（ARI）、腹泻症、麻疹、疟疾与营养不良症。贫穷以及缺乏基本的全民社会服务保障是问题所在。
与妊娠和生育相关的并发症每年导致五十多万妇女死亡，即每分钟的死亡率大于一，同时使更多的妇女受伤和致残。
联合国儿童基金会正与政府部门、人道主义机构、民间与社区领袖、各个家庭与儿童自身一起携手合作找出解决这些问题的方法。以下所列是近期的一些发展：
- 全球发展中国家里，五岁以下儿童的死亡率于1990年至2000年间下降了14%。
- 小儿麻痹症病例从1998年的35万例下降到2001年的仅483例。
- 通过联合国儿童基金会及其合作伙伴赞助的全球碘化食盐运动，碘缺乏病症减少了三分之二。
- 整个90年代免疫率稳定，使70%的儿童得到了预防。但是，在撒哈拉以南非洲国家，免疫接种率常低于50%。
- 计划生育已取得进展，大约7亿妇女，占已婚或处稳定关系的育龄妇女总数三分之二，采用了避孕措施。
然而，在确保每位儿童都有一个最美好的生命开端的道路上，还有许多困难要克服。艾滋病毒/艾滋病的大流行已在全球几个地区造成了灾难性的后果，特别是在撒哈拉以南非洲国家，使几十年在儿童生存与发展方面所取得的努力成果功亏一篑。
性别不平等与歧视仍然存在。导致儿童伤亡的武装冲突还在不断扩散，长期贫穷仍是实现儿童权利的最大障碍。全球一半人口处于极端贫困，其中仍有30亿人每天维持生活的费用不足2美元。
